The "Official" Research Plan forms (which should be included as part of the student report) are available at: http://www.sciserv.org/isef/document/ The Parental Consent and Release Forms are available at: http://www.ohiosci.org/ConsentandReleaseAgreementDec2001.pdf The rule governing review of science projects that include humans or hazardous materials is at: http://www.ohiosci.org/newrule.htm Schools will need to form a Review Committee (Teachers, Principal, Counselor, perhaps) to review those projects that fall within these guidelines. The forms for reviewing these projects are Human Subjects forms. There is also a very useful document at http://www.ohiosci.org/ScienceDayGuide.pdf called the "Science Day Guide". IMPORTANT! Students must present their work as POSTERS ONLY. This means that apparatus and demonstration as part of the student presentation will not be allowed - students must discuss the work done. Logbooks, photographs, graphs, charts, etc. will be allowed at the poster, however.
